If you are incapacitated and unable to communicate for yourself, Advance Directives provide essential guidance on the life-sustaining treatments you wish to receive or decline. These important forms are utilized by medical professionals, caregivers, and most importantly, your Durable Power of Attorney for Health Care to ensure that your preferences are respected.


We offer valuable guidance on end of life planning, including how to select, complete, formalize, and share your Advance Directives, such as your Living Will/Healthcare Directive and your Durable Power of Attorney for Health Care. Additionally, we host workshops and events to help you navigate this crucial process.

Introductory Workshop on How To Plan Ahead

Whether end of life comes naturally, as the result of an accident, or as the final chapter of a disease, planning ahead through end of life planning can make situations easier. It allows you to reflect on your values regarding quality of life before facing critical life and death decisions. By considering Advance Directives, you can understand in advance what measures you would or wouldn’t want taken, depending on the severity of your illness or injury. Most importantly, it empowers you to die on your own terms by sharing your end-of-life wishes with medical professionals, loved ones, and those who support you.


This workshop will provide you with guidance on where to begin with planning and documenting your wishes through Advance Directives. We will also discuss how to have meaningful conversations with those close to you about what quality of life means to you. Additionally, we will answer questions and concerns, offer helpful planning tips, and direct you to free and useful resources available online.
